The worksheet presents visa forms and is great for practicing dialogues on giving personal information for elementary levels. It presupposes pair work on asking and answering questions about the name, surname, age, marital status and place of residence. Tell your students that one of them is supposed to be a customs officer who will be filling in the information about a tourist (another student). Don’t forget to discuss the questions they are going to ask:
- What’s your first name/surname?
- How old are you?
- Are you single or married?
- Where do you come from?
- Where do you live?
You can extend the dialogue by adding the information about the date of birth, children etc.
