This is a present perfect survery that can be used as an introduction for the present perfect tense. I always start present perfect with a class survey. The students are asked to mingle around the classroom and ask each other these questions and find two people that say "YES" to the question. Then when they are all finished asking each other the questions, the teacher asks some of the students post-activity questions like, who has been to England?, etc.
